If you wish to put your child's name down for a term time place at SASCS, please call us to discuss availability first. You will then need to complete both a Registration Form and a Contract.

Registration Form 2017/2018
Please use this form for all after school and/or holiday places for the school year 2017/2018 commencing from now.

Contract & Waiting List Form 2017/2018
Please complete this form if seeking to put a child's name on the waiting list for a term time place (call first to discuss availability).

Please complete one form per family for each school year.

Record of Medication - in the event a child requires ANY medication to be administered during their time at SASCS, parents/carers will be required to complete the attached medication record.
